1 month - Hep B
2 months - Pentacel (DtaP, Hib , Polio) Prevnar, Rotateq
4 months - Pentacel (DtaP, Hib , Polio ) Prevnar, Rotateq
6 months - Pentacel (DtaP, Hib , Polio ) Prevnar, Rotateq
9 months - Hep B
12 months - MMR, Prevnar
15 months - Varivax, Hep A
18 months - Pentacel (DtaP, Hib, Polio)
2 years - Hep A
3 years - None
4 years - MMR, Varivax
5 years - Quadracel (Dtap, Polio)
11-12 years - TdaP, Meningitis, HPV
16 years - Meningitis 
College age - Td, Bexsero

* Well Child Checks are recommended annually from 3 years of age and older, regardless of whether vaccines are due at that time.

**Flu shot is recommended annually.  Two shots are needed one month apart if less than 9 years old and they’ve never received a flu vaccine.
